Step 1: Call to set up your free 15-30 minute initial consult where we will discuss what you're looking for and what your needs are. Once that is complete, if we choose to work together, the 'Plan of Support' Setup Package can be purchased.

 Full consultation, signing of contract agreements & waivers, research, written (non-medical) plan, and follow-up to determine which package fits your needs and will best fit your 'Plan of Support.' Includes:

       

    •    Intake virtual/in-person meeting (60–90 minutes)

    •    Plan of Support document + next steps checklist

    •    resource shortlist

    •    1 plan review call (30–60 minutes)

Step 2: Involves choosing a Retainer Package that fits your needs/budget after the 'Plan of Support' is created:

Package A: Priority Retainer 

              (Highest Support):

High-touch support for complex or time-sensitive situations. Includes more frequent contact, faster response times, and intensive coordination. 

 

​       Includes per month:

• 4 scheduled phone/virtual calls (up to 45 min each)

• 9 coordination call requests/month with follow-up

• 1 in-person/virtual family meetings (60 min each) (For client and POA/decision-maker only)

• 4 plan check-in and updates with family or client via phone/email per month (15 min)

• Priority scheduling + faster turnaround

• Intensive coordination and frequent follow-up

• Support during high-change periods (care transitions, rapid decline, complex logistics)

• Ongoing advocacy-style communication support (non-medical)

• Monthly summary

Best for: complex or time-sensitive situations needing frequent contact.​

 

Package B: Standard Support Retainer (Medium Support):

For families who want consistent help implementing the plan. Includes regular check-ins, coordination tasks, and support between decisions and appointments. ​

 

    Includes per month:

• 3 scheduled phone/virtual calls (up to 45 min each)

• 6 coordination call requests/month + follow up

• 1 in-person/virtual family meeting (60 min) (For client and POA/decision-maker only)

• Ongoing care planning support and plan adjustments

• 2 plan check-in and updates with family or client via phone/email per month (15 min each)

• Coordination of next steps (calls/emails with providers)

• Appointment prep support

• Follow-up tracking after decisions/appointments

• Monthly summary

Best for: Families who want consistent help implementing the plan.

     Package C: Maintenance Retainer           (Lowest Support):           

 Call for Pricing 

 

​Light ongoing support to keep the plan organized and on track. Includes periodic check-ins and limited coordination as needed.

         

  Includes per month:

• 1 scheduled phone/virtual call (up to 30 min)

• 4 coordination call requests/month + follow up

• 1 in-person/virtual family meeting (30 min) (for client and POA/decision-maker only)

• 1 Plan check-in and update with client or family via phone/email per month (15 min)

• Basic coordination support 

• Simple resource sharing and next-step guidance​

• Monthly summary

Best for: families who want light support and periodic check-ins and for families who want continued services after having stepped down from a higher level of support Retainer level.

Additional Billable Time:

Requests beyond the scope of the selected package/retainer are billed per hour  (billed in 15-minute increments) and include additional phone calls, meetings, or any services provided. You will only be billed additional hours for requested and approved additional time each month based on my availability.
